Testosterone and male hormone tests

A male hormone test can look at how much testosterone is circulating, how the blood carries it and which signals regulate its production. Total testosterone is one part of the assessment: SHBG and albumin make it possible to estimate the free fraction, while FSH and LH add information about hormonal regulation.

How to choose your testosterone test

This is not medical advice and does not replace a consultation: it is the reasoning I would use to choose, in case it helps you decide which one to order.

For a first assessment, the total and free testosterone test includes total testosterone, SHBG and albumin. With those results, free testosterone can be calculated using a validated formula.

If low testosterone has already been confirmed and you want to investigate its cause, the low testosterone assessment adds FSH, LH and prolactin. The full male hormone blood test also widens the metabolic and general context.

When gynaecomastia has already been identified, the gynaecomastia assessment brings together the hormones that are specifically reviewed for that problem.

During testosterone therapy, the monitoring profile checks both hormone levels and safety parameters. It is not designed to diagnose a deficiency before starting.

Total testosterone, SHBG and free testosterone

Total testosterone includes the hormone bound to proteins and a small free fraction. SHBG can change with age, weight, thyroid function, liver disease and other circumstances. That is why two people with the same total testosterone can have a different free fraction.

Free testosterone is estimated from total testosterone, SHBG and albumin. This calculation adds context when total testosterone is not enough, but it still depends on a properly taken sample and on a clinical assessment.

The time of day and repeating the test are part of the assessment

Testosterone varies during the day and is usually higher in the morning. A single low value is not enough to confirm hypogonadism: it is checked with a new morning measurement and interpreted together with symptoms, medical history and medication.

Once the deficiency has been confirmed, FSH and LH help investigate whether the cause lies in the testis or in its regulation. That classification belongs to the diagnostic process and should not be inferred automatically from a result ordered online.

What these profiles do not assess

They are not male fertility tests and do not include a semen analysis. Nor do they decide on their own whether to start testosterone, which dose to use or how to change a treatment. The monitoring profile provides data for a clinical review, not an individual treatment plan.

Before your blood test

Have the sample taken in the morning. The profiles require at least 8 hours of fasting and the broadest one may require up to 12; you can drink water.

Avoid alcohol and intense exercise the day before. An acute illness, little sleep or hard training can temporarily alter some measurements.

If you already use testosterone or another hormone treatment, follow the sampling time given on the profile page or by whoever manages your treatment. Do not change the dose or timing on your own.

Frequently asked questions

Should testosterone be measured in the morning?
Yes. It varies over the day, and a morning sample allows the result to be compared under more stable conditions.
Does low testosterone confirm hypogonadism?
Not on its own. It is usually confirmed with another morning measurement and assessed together with symptoms, medical history and medication.
Why are SHBG and albumin measured?
Because they affect how much testosterone is carried in the blood and make it possible to estimate free testosterone.
Can these profiles be used to assess fertility?
No. They can provide hormonal information, but assessing male fertility requires other tests, including a semen analysis.
Can the therapy monitoring profile be used before starting testosterone?
No. It is designed to monitor a treatment that has already started. The assessment beforehand has other aims and uses different profiles.
